Annual Committee Report

PAS Committee Chairs should complete the following form and have it submitted no later than January 15 of each year. This form contains short answer, long answer, and file upload options for your Committee Meeting agenda and minutes.

PASIC Meeting

Provide details about your more recent meeting during PASIC.

Committee Members Present:
Use the "return" key to provide a list of the committee members that were present at your PASIC meeting. If a list is provided in your minutes, you do not need to include a list here.
